To make an appointment, we kindly request of you to provide your insurance information so that we may verify coverage for you, prior to your initial session. If the appropriate form is not filled out, your scheduled appointment will not be approved.

If going through insurance and booking, you MUST provide your insurance information, so that we may verify your own eligibility.

You may have out-of-network benefits for nutrition counseling! For those clients, payment is required at the time of service. Some of these clients have received reimbursement for a portion of paid fees. After our sessions, you will receive two receipts on your client portal account, one for your credit card and a Superbill receipt that is required for reimbursements to your insurance company. Depending on your plan, you may need to submit both receipts in order to request reimbursement.

Your credit card is kept on file in order to secure your appointment.
Your credit card will be charged to cover the cost of your session (if not going through insurance), additional non-covered services (group programs, journal reviews, phone calls, responses to nutrition-related messages between sessions, any co-payments, deductibles or any amount insurance does not cover per session), or any late cancellation fee ($50 for no-shows, 10+ minutes late, or less than 48-hours notice to reschedule/cancel).

If you are going through insurance:
I provide care to clients located in other states, depending on the laws of those states. This also means that I can submit claims to network insurance companies for clients in other states.
Before our work begins, I will need to check for updates to your state regulations.
If you are going through self-pay, I can accept you regardless of being out of state.
